How to Get a Prescription:
- Log into your patient portal or electronic health record such as MyChart
- Send a message or call your primary care provider to request a prescription
- Prescriptions may be issued for:
- Lifetime Need
- 99-Month Duration
- These types of prescriptions can be used for ongoing supply needs
Prescription Should Include:
- Patient’s full name
- Physician’s name and contact info
- Physician’s signature
Diagnosis
- A statement of medical necessity (e.g., “CPAP Machine with Mask”)
- For CPAP/BiPAP: Pressure settings (used to program your machine)
